New Patients
Welcome.
This practice provides evidence-based, whole-person care for women navigating hormonal transitions.
All new patients begin with a referral from their primary care provider before scheduling an initial consultation.
This ensures care remains collaborative and well-coordinated within your existing healthcare team.
How to Get Started
Step 1: Obtain a Referral
Ask your family physician, nurse practitioner, or specialist to send a referral to Dr. Shannon Lockhart’s practice. Referral form available here.
Once received, our office will contact you via email to invite you to book an initial consultation online.
Step 2: Book an Initial Consultation
Once your referral is processed, our office will contact you via email to invite you to book an Initial Consultation visit online.
Fees are required for this visit:
• $350/h pro-rated to length of visit (Initial consultations generally 60 mins)
Virtual appointments are available across British Columbia.
Your first visit includes a detailed review of your medical history, systems, and goals.
Step 3: Complete Intake Forms
You’ll receive secure intake forms through our platform before your appointment.
Please include current medications, test results, and your primary concerns to help guide our discussion. It is essential we receive these a week before your first visit to provide time for appropriate review.
Step 4: Your First Visit
We’ll explore potential root causes, outline next steps, and create a practical, evidence-based plan.
Follow-up may include private visits or MSP-covered group medical series, depending on how your health challenges align with current GEMS (Group Education and Multimodal Support) sessions being offered.
What to Expect
Comprehensive, in-depth assessment integrating conventional and functional medicine to address the root-causes of your conditions
Collaborative, educational care that complements your family physician’s work
Clear documentation and communication back to your referring provider, and to you if you choose.

FAQs
Do I need a referral?
Yes. All new patients require a referral from their family doctor, nurse practitioner, or specialist to initiate care.
Do you replace my family doctor?
No. This practice provides consultative care that complements your primary provider.
Are labs or supplements included in the consultation fee?
No. But most of our testing and supplement recommendations are evidence-based and often covered by MSP.
Why are the intake forms so long? How does the data you collect support women’s health?
I collect detailed information because women’s midlife symptoms are often overlooked, under-recognized, and under-researched. A comprehensive intake helps me understand what’s driving your symptoms and tailor your care from the start, while de-identified trends allow me to advocate for better access, funding, and recognition of women’s hormonal health at a system level. Your information supports both your own care and broader change for women across BC.
What is the cancellation policy?
Private appointments require 48 hours’ notice for changes or cancellations. We provide several reminders via text or email prior to appointments.
